Shoulder Barbotage is a procedure used to breakdown and remove calcium buildup in the rotator cuff tendons of the shoulder, which can cause pain and stiffness. Calcific tendinosis, is a condition affecting the rotator cuff tendons of the shoulder. It is caused by a build-up of calcium within the tendons of the shoulder.
Guided by ultrasound, Barbotage is a very effective treatment of calcific tendinosis where the procedure involves breakdown and removing the calcium buildup in the rotator cuff tendons of the shoulder.

Shoulder Barbotage is a relatively short procedure typically performed by a radiologist consultant. The consultant will first conduct an ultrasound scan of your shoulder to locate the calcium deposits. Local anesthetic is used to numb the area. A fine needle is then inserted into the calcium to break it up and saline fluid may be used to help draw the calcium into a syringe. Often, steroids are also injected into the shoulder to help reduce any inflammation causing pain.
The entire procedure typically takes around 20 – 25 minutes.
There are numerous studies which have repeatedly demonstrated and confirmed that needle placement into target structures have a greater degree of accuracy and are successfully placed within the target structure on a much higher percentage of attempts when performed with ultrasound guidance. During the procedure, the needle’s position can be monitored in real-time after it enters the body tissues, allowing for accurate needle placement and injection delivery.
For most patients, the benefits of shoulder barbotage outweigh the risk of any side effects. The decision to use shoulder barbotage is made carefully by GP’s or Consultants based on your diagnosis and medical history. Overall, 80-90% of patients experience significant benefits from the shoulder barbotage procedure, enabling them to return to their normal daily activities within a few weeks. However, if you have a large calcium deposit in your tendon, it may not be fully removed in one session and you might need a follow-up procedure.
