What is Skin Prick Testing?

Skin Prick Testing (SPT) is a quick and effective method used to identify potential allergies. It helps determine whether an individual is likely to have an immediate-type (IgE-mediated) allergy or no allergy at all. In some cases, results may fall into an indeterminate range and require further assessment within an NHS setting.

SPT is generally straightforward and minimally uncomfortable. Small drops of allergen solutions are placed on the skin, along with both negative and positive controls to ensure the test is valid. When commercial extracts are not available, fresh foods may be used; this is known as prick-prick testing.

How is Skin Prick Testing Performed?

Testing is usually carried out on the forearm with the palm facing upwards. The skin is marked with numbers to identify each allergen. A small drop of each allergen is placed next to the corresponding number. A sterile lancet is then used to gently prick the skin through the drop. This may feel slightly uncomfortable but should not be painful. The drops are gently blotted away, and the results are assessed after 15–20 minutes.

A raised, itchy wheal similar to a nettle rash may appear at the test site. It is important not to rub or scratch the area, as this can affect the accuracy of the results. Although rare, a more significant allergic reaction can occur. For this reason, testing is carried out by our trained staff who can provide appropriate treatment and monitor recovery if needed.

Is Skin Prick Testing Safe?

Skin prick testing is generally well tolerated. The procedure may cause mild itching or temporary redness if a reaction occurs, but it is not considered painful.

The test is clinically approved and performed in a controlled medical setting by experienced specialists, making it a safe and reliable diagnostic tool.

How long does skin prick testing take?

The test itself takes around 30-45 minutes. Results are usually available during the same appointment, as reactions typically appear 10-15 minutes.

What medications should be stopped before this test?

The following medications/ointments/creams should be discontinued prior to having a skin prick test as they may affect the test results:

Cough medicines (containing antihistamines) 5 days

Antihistamines 1st generation (e.g. Chlorphenamine – Piriton, Promethazine – Phenergan, Alimemazine – Vallergan,Hydroxyzine – Atarax/Ucerax, Ketotifen – Zaditen) 5 days

Antihistamines 2nd generation (e.g. Loratadine – Clarityn,Cetirizine – Zirtex, Fexofenadine – Telfast) 7 days

Topical Tacrolimus (Protopic), Pimecrolimus (Elidel) 7 days

Topical steroids (mild) – to forearms (e.g. hydrocortisone) 1 day

Topical steroids (moderate to potent) – to forearms (e.g. Clobetasol – Dermovate, Mometasone – Elocon, Fluocinolone – Synalar, Betamethasone – Betnovate, Fluticasone – Cutivate, Clobetasone Butyrate – Eumovate) 2 weeks

Oral steroids (e.g. Prednisolone) (Further discussion with consultant if using long term) 1 Day

Omalizumab (Following discussion with consultant) 4 weeks

Please Note: testing will not be completed if child/young person has had a reaction within the previous 6 weeks.

Do I need to stop asthma medications?

Do not stop any inhaled medication. If your child is taking a short course of oral steroids for an acute episode of asthma, the test will need to be re-arranged.

What should we do if our son/daughter cannot withhold these medicines?

Is skin prick testing painful?

The test is not considered painful. You may feel a slight scratch on the skin and if a reaction occurs, mild itching or redness may develop temporarily.

Can skin prick testing detect food allergies?

Yes. Skin prick testing can assess many common food allergens. However, the most appropriate test will depend on your symptoms and clinical history.
